With JEE (Main) Session 2 just a couple of days away, April 2-April 8, 2026, the preparation has entered its final phase. At this stage, knowing the syllabus is not enough. Following the required exam-day procedures matters just as much. Among the essential documents, the Self-Declaration or Undertaking form holds particular importance. It appears on the first page of the admit card and is mandatory for entry into the exam centre. Any mistake in filling it, or failure to carry it, can delay or even disrupt entry.
Understanding the self-declaration form
This form serves as a formal confirmation from the candidate. By completing it, the student agrees to follow the National Testing Agency’s rules related to exam conduct, safety, and health guidelines. It also confirms that the candidate is physically fit to appear for the test.
How to fill it correctly
1. Check that all details on your admit card match your registration. Fix any errors before the exam.
2. If you have had no symptoms in the past 14 days, leave the health section blank.
3. Fill travel history only if required and applicable.
4. Paste a passport-size photo, preferably the same as your registration image.
5. Add a clear left-hand thumb impression using proper ink.
Do not sign in advance. Sign only at the exam centre in front of the invigilator.
Common errors to avoid
· Signing the form at home is a common mistake and may require a fresh printout.
· Ensure your thumb impression is clear to avoid verification issues.
· Carry all pages of the admit card, not just one.
· Use the same photograph as on your admit card.
· Only printed copies are accepted, so keep an extra as backup.
